Healthcare Provider Visits

Our team includes a dedicated MD who oversees residents' chronic conditions using remote patient monitoring with medical devices. This proactive approach helps prevent medical issues such as hypertension (HTN), diabetes (DM), chronic obstructive pulmonary disease (COPD), and congestive heart failure (CHF) from escalating, thus minimizing hospitalizations and emergency room visits. We collaborate closely with residents' Primary Care Physicians (PCPs) to coordinate physical therapy through home health services and scheduled PCP visits. Additionally, we offer Telemedicine consultations during work hours, providing convenient access to the MD for on-demand medical advice.

Medication Management

Our Registered Nurse, in conjunction with the MD, offers consultations and training on medication administration. We utilize an electronic chronic care management system to track residents' medications and care plans. This system allows the pharmacy on record to swiftly and accurately adjust medications as needed through our Electronic Medical Record (EMR) system. We also employ e-prescribing methods to deliver medications to residents' preferred pharmacies or for home delivery.
